• Supportive and collaborative team environmentPosition SummaryAs a Licensed Vocational Nurse (LVN), you'll provide direct, in-home patient care under the supervision of a Registered Nurse (RN). You will play a key role in implementing care plans, educating families, and supporting patients throughout their health journey.Key Responsibilities
  • Provide direct patient care in accordance with the State Nurse Practice Act
  • Follow and implement the plan of care as set by the clinician
  • Accurately document patient care in a timely manner
  • Educate patients and families about diagnoses, treatments, and home care needs
  • Coordinate with team members to ensure all patient care needs are met
  • Ensure efficient and responsible use of equipment and supplies
  • Participate in ongoing professional development
  • Submit to monthly supervision by an RN per agency policyQualifications
    • Graduate of an accredited LVN or practical nurse program
    • Current LVN/LPN license in the State of California
    • Minimum of one (1) year nursing experience (preferred: home health, hospice, hospital, or facility care)
    • Strong verbal, written, and organizational skills
    • Current CPR certification (BLS)
    • Valid driver's license and insured vehicle in good working condition
