Job Description:
A company is looking for a Clinical Care Specialist.
Key Responsibilities
Manage and monitor a patient census, coordinating Chronic Care
Management services for chronically ill patients
Create individualized Care Plans and track patient progress,
including labs, vitals, and medications
Conduct care coordination and provide patient education while
collaborating with the care team and families
Required Qualifications
Registered Nurse (RN) with an active compact license
Five years of nursing experience required
Strong computer skills and reliable home internet connection
Experience with chronically ill patients and telephonic case
management preferred
Self-motivated and independent worker
