Job Description:
A company is looking for a Clinical Pharmacist Patient Care -
Remote.
Key Responsibilities:
Conduct telephonic medication consultations to meet/exceed
production and quality metrics
Complete pharmacotherapy assessments and develop plans to improve
medication adherence
Document clinical consultations and participate in staff
development opportunities
Required Qualifications:
Active, unrestricted pharmacist license in the state of primary
residence
5+ years of pharmacy practice experience, including 2+ years in
Clinical Pharmacy Practice
Experience with Medicare STARs & HEDIS Measures, including
Medication Therapy Management
Proficient with Microsoft Office applications
Ability to manage multiple projects in a rapidly changing
environment
